Abacus Accounting Combined Matters Cover Pages

When running the Combined Matters Cover Pages, a blank row column is now in place where previously it held the trust transfer option. There should be a way to customize the options for Combined Matters Cover Page similar to how the billing format codes option works in the prebills/print bills.
